How do I add states to my appointment?

I want to submit business in states that are not included in my appointment.

If you are Symmetry and in a JIT state, click here. Otherwise, follow the steps below:

If you want to submit business outside the state(s) in which you are appointed, you must add that state to your appointment before submitting business. Activating your new appointment can take 24-72 hours and involves two steps.

STEP 1

Please email SBLI directly at quilityagentservices@sbli.com with the subject line: ‘Quility – Agent Contract Code – Agent name – New Appointment’. Example: Subject: Quility – 12345 – Michael Smith – New Appointment Body: Please appoint me to sell in the following states: AB, CD, EF

STEP 2

Once your appointment is complete, you will receive an email from SBLI Then your appointment will be activated within the eApp for QLT. You can test whether your appointment is activated by requesting a quote in the new state(s). The eApp will return an error [Agent ineligible for this product] if your appointment is not yet active. Alternatively the eApp will return prices if your new state is active.
